Procedures

1. Referrals: Applicants must be referred by health care providers, social service providers or other medically related organizations.

2. Restrictions: Financial assistance would be limited to $2,500 per applicant per year or a $7,500 lifetime per child limit. Additional funding may be given with extenuating circumstances and Board of Director approval.

3. Funds: The amount of funds granted to an individual organization will be approved at the discretion of the Celebrities Fore Kids board of directors.

4. Reporting to the Board: The social worker or key representative of the approving organization will report annually to the board of directors of Celebrities Fore Kids, Inc. providing a summary of distributions as directed by the board.
