Qualifications for Participation

1. Age: A child must be 18 or under as of date of their application for assistance.
Parents diagnosed with cancer can apply for assistance if they have children 18 years or younger.

2. Diagnosis: Applicants must be diagnosed with some form of cancer,
reside on the Treasure Coast, and being actively treated for the disease.

3. Need: Applicant's need must be a direct result of child's or parents current condition and will be subject to Federal Poverty Guidelines (extenuating circumstances will be considered). Examples of need can include, but are not limited to, transportation to and from treatment centers, food and lodging during treatment, emergency funding for daily living needs, medical/prescription costs (not covered by insurance), and individual and/or family counseling.

4. Extended Assistance: If a patient for more than one year, a more intensive application
process and review may be requested. Any special case may be brought before the
Celebrities Fore Kids Board of Directors for additional consideration.
